On July 25th, 2026 West Bradenton Baptist will host a “Day 4 Hope” to help kids get ready to start the new school year. This event is by invitation only. If you and your school age child wish to participate, you must fill out this registration form.

“Day 4 Hope” includes these FREE items for your child:
– BREAKFAST or LUNCH
– MEDICAL EXAM FOR SCHOOL
– DENTAL CHECK-UP
– HAIRCUT(basic hair cut only)
– PHOTOS
– CARE BAGS FILLED WITH PERSONAL HYGIENE ITEMS
– BACKPACK FILLED WITH SCHOOL SUPPLIES
– FRESH PRODUCE & NON-PERISHABLE GROCERIES
– PRAYER
– CHILDREN’S ACTIVITIES

Before the event an email or text will be sent to the address and number you list on this form. This will confirm your registration and designate a specific time of arrival for your family at the event. You must bring that confirmation with you the day of the event.
