Awaken Church Day4Hope

On July 22nd, 2023 Awaken Church will host a “Day 4 Hope” to help kids get ready to start the new school year. This event is by invitation only. If you and your school age child wish to participate, you must fill out this registration form.

“Day 4 Hope” includes these FREE items for your child:
– BREAKFAST or LUNCH, SNACKS
– MEDICAL EXAM FOR SCHOOL
– DENTAL CHECK-UP
– HAIRCUT
– PHOTOS AND SHERIFF ID CARDS
– GIFT CARD FOR AND/OR NEW PAIR OF SCHOOL SHOES
– GIFT CARD FOR AND/OR NEW SCHOOL CLOTHES
– CARE BAGS FILLED WITH PERSONAL HYGIENE ITEMS
– BACKPACK FILLED WITH SCHOOL SUPPLIES
– NON-PERISHABLE GROCERIES
– PRAYER
– CHILDREN’S ACTIVITIES

Before the event a ticket/letter will be mailed to the address you list on this form. This will confirm your registration and designate a specific time of arrival for your family at the event. You must bring that confirmation with you the day of the event. Thank you and we look forward to seeing you in July.
